Thank you for your interest in the TRiO EOC College Connections Project.

You must be age 19 years or old and a resident of New York City to participate in this program

Before services are received, the Participant Form must be completed and reviewed by a College Connections Project staff.

Participants under the age of 18 must complete the

1. New Participant Form and

2. Your Parent/Guardian must complete the Parent Consent form.

Note: After application is received, you will be contacted by a program staff within 1-2 working days to schedule an Introduction meeting
